INSTRUCTIONS
1-) My client …, … with the insurance registration number S.S.K.it is also registered in . My client … has paid premiums from the Optional Insurance arm of the Insurance Directorate as of the date of /…/.
2-) My client has applied to the defendant institution to retire with a petition with a date and registration number /…///, the institution has informed them in an answer letter with a day and … /…/// that they do not have such a file, my client’s request has been rejected, so it is not considered Voluntary Insured.
3-) The respondent institution initially accepted my client’s request to become an Optional Insured, …/…/… – …/…/… as dec has collected all premium payments during the period between the dates, it has never made any warning that the premiums paid are invalid.
4-) … The Insurance Directorate, … /… /… the premiums that were not paid retrospectively with the date and … receipt were collected with the printed “Optional Insurance Pirim Collection Receipt” together with the delay increase. For these reasons, it has become mandatory for us to file a lawsuit to determine the optional insurance of my client, whose pension claim has not been accepted.
